Applying for “LOST” Dog’s Registration and Pedigree Certificates from PCCI

Many of you here are asking on how to apply Dog’s Registration and Pedigree Certificates if ever it was lost, stolen or what have you. So I hope this article will answer your queries regarding this matter, it was taken from the PCCI website itself, read:
I lost my dog’s Registration and Pedigree Certificates. How do I obtain another one?
You can download the “AFFIDAVIT FORM” from our website. Then you will have to fill-up and sign the form and send it to the office along with your payment for the Duplicate Copy amounting to Php110.00
I just lost my dog’s PCCI certificate that I obtained from the previous owner and I did not transfer the ownership into my name. How do I obtain another certificate?
If you lost your dog’s certificate and the dog was not yet transferred into your name, the dog’s recorded owner will need to fill-up the PCCI Affidavit Form and the Supplemental Transfer Statement Form. Please include the following information:
o Dog’s name, registration number and breed
o Recorded owners name, address and signature
o Php110.00 processing fee (you can either pay by check or money order made payable to the PCCI.
o Php250.00 transfer of ownership
o Php33.00 supplemental transferIf they do not have the dog’s name and registration number please provide us with the following in its place:
o Names and registration numbers of the sire and dam, and breed
o Date of birth of the litter
o Recorded owner’s name and addressWhen the required information is received a new certificate will be mailed to you as the new owner.
Please remember a duplicate certificate can only be requested by the current owner on PCCI record.
